MyCommunity Health is a patient portal that connects you to a portion of your personalized health information from the privacy of your home at any time, day or night using a secure, online patient portal.

Please note for patients ages 12-17, the portal will only display limited information in order to protect patient privacy. Patients and their proxy users (parents) will have access to see immunizations, the ability to message providers and request appointments.

Locate medical record number (MRN#) on a past billing statement (listed as your account #), a recent visit summary, call your Community Physician Group provider office or you may call (406) 327-4100 to be connected to a patient access specialist during business hours.
Ask us at your next visit or stop by Guest Services at the main hospital entrance.
